Complete an RNZCGP pre-approval form

Before you can start working on your CQI project, it needs to be approved.

Download the CQI pre-assessment project outline form from the RNZCGP website.

Follow the guidance below to make sure you include all the information your assessor is looking for.

Once complete, submit it to an RNZCGP-approved assessor.

What does your practice want to achieve from the project, including baseline to end-of-project projections?

 

Write an aim statement that includes:

  • outcome
  • metric(s)
  • baseline
  • target
  • timeframe

What is your realistic timeframe for completing the project (including date for assessment), and suitable clinical leadership?

 

Give the date you expect to have your report ready for assessment.

You might need to estimate this. Consider the time needed to carry out the project, analyse data, and write the report. Be realistic.

 

Describe how the practice will incorporate the NZ Triple Aim elements.

 

Write a description for each of the three aims of the NZ Triple Aim:

  1. Improved quality, safety, and experience of care.
  2. Improved health and safety for all populations.
  3. Best value for public health resources.

Describe the type of evidence or data used to identify your initiative, including your data source.

 

Write a rationale for your project.

It should include a description of the data source(s) you used.

Which quality improvement methodology has the practice team agreed upon?

 

Explain which quality improvement methodology you have chosen.

Which quality improvement tool(s) have the practice team agreed upon?

 

State which quality improvement tools you think you'll use to display your data.

You can just say "charts or graphs", "tables" etc. You don't need to be any more specific.

Does your chosen methodology have capacity for multiple cycles?

 

If you've chosen a methodology that has capacity for more than one cycle, you can just say "yes" on the form.

Describe how your CQI initiative is scalable and sustainable.

 

Explain how your project is scalable and sustainable.

You need to describe each element separately.

Once the pre-approval form is complete, engage a CQI assessor and submit the form.

Amend and resubmit the form if your assessor asks.
