Outcomes

Identify all the outcomes – changes or improvements to patient care and/or service delivery – that have happened because of your project.
Outcomes make up the final section of your Cornerstone CQI report. They should be communicated to all of the project's stakeholders – internal and external.

Document all project outcomes
What have you achieved?
Think about:
- changes to health outcomes
- reduction of inequities
- better access to services
- changed behaviours or attitudes
- wider whānau benefits
- improved skills.
Did you achieve what you expected to, under the New Zealand Triple Aim?
Be honest about the achievements so that you can continue to identify where further improvement is needed.
Identify lessons learned
What would you do differently next time?
Think about areas where you could improve your processes for future projects:
- How did the team
work together? For example:
- Did people do the things they were supposed to?
- Did the work schedule go to plan, or did it take more staff than envisioned?
- Were the team engaged or reluctant?
- Did you choose the right people?
- Did you gather the right information?
- What challenges were there?
- How much did the project cost in resources, and time?
Decide the next steps
How could you run another PDSA cycle of this project?
For example:
- How could you scale up the project or expand it to benefit more people (essential for a Cornerstone CQI project)?
- Could you repeat the project with a different group of people?
- How could you embed the changes so they become a standard process at the practice, e.g. in increments, or all at once?
You don't need to actually do these things for this Cornerstone project, but you need to identify what the next steps could be.
Share the outcomes
How did you communicate the outcomes to staff, patients, and other stakeholders?
It's important to communicate your success, particularly to anyone who has contributed or been affected.
For example:
patients and whānauFor example:
- put information on practice noticeboards
- write about it in your newsletter
- create a display in the waiting area
- add something to your website
- post on social media.
practice teamFor example, you could:
- present your results at team hui
- plan a team morning tea
- display your results in the staffroom.
other stakeholders For example, you could:
- invite stakeholders to a korero with kai to share your successes
- go to see them
- send information emails.
Sum it all up
Overall, how did it go?
Write a conclusion – a paragraph or two to summarise the outcomes.
Think about:
- Did you achieve your initial aim?
- Did the outcomes you identified align with the NZ Triple Aim?
- Do you consider the project a success?
- What can you take away from the experience?
